An endoscope is a fiber optic medical instrument designed to provide clinicians with real-time visual access to internal anatomical cavities, lumina, and organ systems. At its fundamental physical core, medical endoscopy relies on optics and optoelectronics to transmit light deep into human tissues and capture high-fidelity optical signals for diagnostic and therapeutic interventions. Fiber optic light transmission operates on the optical principle of Total Internal Reflection (TIR). A glass optical fiber consists of a ultra-pure silica or glass core surrounded by a cladding material with a lower refractive index (n1 > n2). When light rays enter the core within a specific critical acceptance angle (determined by the Numerical Aperture), the light undergoes complete internal reflection along the fiber shaft with minimal attenuation.
In illumination systems, non-coherent bundle configurations are utilized to transfer cold light from external LED or Xenon light sources directly to the distal end of the insertion tube, illuminating tissue microvasculature without thermal trauma.
Legacy fiber optic endoscopes utilize coherent fiber optic bundles where tens of thousands of individual glass fibers are precisely aligned to transfer optical images dot-by-dot from the objective lens to an eyepiece sensor. The insertion tube of a flexible endoscope is a multi-layered engineering marvel. It comprises an inner flexible snake-bone mechanism driven by stainless steel pull wires for 4-way or 2-way distal articulation, wrapped in high-tensile stainless steel braid, and coated with an outer medical-grade polymer sheath (such as Pebax or Polyurethane). This sheath must exhibit chemical resistance to disinfectant solutions (e.g., Glutaraldehyde, Peracetic Acid), zero toxicity, extremely low surface friction, and precise torque transmission capabilities for seamless navigation through tortuous lumens like the ureter or bronchial tree.
